Especially for non-BLEACH readers, please read this XD;
First things first: Appearance:
Normal
Karakura High uniform
Shinigami
Bankai, with Tensa Zangetsu

Attacks:
As Zangetsu is perpetually in his shikai form ( like a level 2 attack form), Ichigo can translate his spiritual pressure into real, tangible energy. In other words, it just acts like a normal sword, albeit a very big one. Zangetsu is as tall as Ichigo is.
Ichigo's bankai (level 3 attack, highest power) causes him to change appearance (see links above). In bankai form, he has an incredible speed boost, allowing him to perform attacks from both sides almost simultaneously.
Aside from these little add-ons, Ichigo's basically your hack/slash kinda guy. :D
Damage:
In shinigami form Ichigo's only present in konpaku (spirit) form. His physical body is... somewhere else XD; but it is possible to deal damage to him with magic or something else that will attack the spirit instead of a physical body. For example, swinging a huge sword down on him will just result in the blade passing right through him, but hitting Ichigo with a blast of Ultima is a different story altogether. Any wounds that Ichigo takes in konpaku form will materialize on his physical body once he returns to it, if he is not healed.
Other random stuff that you guys should know:
In shinigami form, he is invisible to the residents of Paixao, save for those who were originally not from the place. As for the others, sensitivity can range from merely sensing his presence, detecting faint outlines, to seeing him as they would a normal person. That depends on the character's sensitivity to spirits and I will leave that to be entirely up to you guys ^_^ the only exceptions would be other shinigami.
Finally:
Hollow!Ichigo
Hollow!Ichigo appears when Ichigo puts too much strain on himself and fails to suppress the Hollow within him that was created as a result of Urahara Kisuke's "training", and is recognized by the half of a Hollow mask that appears on the left side of his face (which will attempt to reform itself), as well as the changes in his eye color. During this time, they may have arguments with each other and stuff ^^; (This means that Ansem's not the only crazy guy talking to himself!) Ichigo normally is able to resume control of his body almost immediately and pull the mask fragments off [as seen here]. Although Hollow!Ichigo is usually just an unwelcome interruption, it may become a problem if Ansem decides to take control of Ichigo via the Hollow inside him.
Oh, and I'd like to apologize if parts of my above post aren't according to canon; I haven't read those chapters of BLEACH for a long time and I'm having a helluva time trying to download them again for a reread. T__T and also, sorry that I didn't post this earlier. ><;
